Plan Options

Plan A

  • Pays $250,000 if your death is caused by any kind of covered accident.
  • With the added Family option, plan pays $125,000 for your spouse and $5,000 for your children (not available in ID).
  • Amounts double for fatal accidents due to riding as a fare-paying passenger inside a Common Carrier with regularly scheduled routes, like a plane, train or bus.

Plan B

  • Pays $125,000 if your death is caused by any kind of covered accident.
  • With the added Family option, plan pays $75,000 for your spouse and $5,000 for your children (not available in ID).
  • Amounts double for fatal accidents due to riding as a fare-paying passenger inside a Common Carrier with regularly scheduled routes, like a plane, train or bus.

Included features

  • No automatic policy termination—coverage amount stays the same up to age 69. At age 70, your coverage amount reduces by 50%.
  • Monthly payment guaranteed to stay the same for the first year. After that date, rate adjustments—if necessary—are limited to once in a 12 month period.
  • Benefits are paid if you die as a direct result of an injury due to a covered accident within 90 days of that accident (180 days in UT and 365 days in WA).
  • Common Carrier benefits are paid for a fatal injury that was a direct result of a collision, crash or sinking of a duly licensed Common Carrier, which means a public conveyance (like a plane, train or bus) which is provided and operated for regular passenger service by land, water or air.

Exclusions, or “What is not covered”:

For group coverage in the states of AZ, DC, DE, FL, HI, MA, MO, NJ, VA and WY the exclusions are as follows (Form Number:GC560/GR972 Series): No benefit shall be paid for Loss or Injury that is caused by, results from or contributed to by:

  • An intentionally self-inflicted Injury, suicide, or any attempt at suicide, while sane or insane (while sane in MO);
  • Any active participation in a riot, insurrection or war, either declared or undeclared;
  • The Covered Person’s taking or using any narcotic, barbiturate or any other drug or medication, unless taken or used as prescribed by a physician;
  • The Covered Person’s blood alcohol level being .08 percent weight by volume or higher;
  • The Covered Person operating or riding in any kind of aircraft, except as a fare-paying passenger on a regularly scheduled commercial flight;
  • The Covered Person committing or attempting to commit a felony or an assault or being engaged in an illegal activity;
  • Sickness, disease, bodily or mental infirmity or their medical or surgical treatment including diagnosis (except bacterial infections which result from an Injury) or mental disease or disorder;
  • Voluntary gas inhalation or poison voluntarily taken, administered or inhaled;
  • Taking alcohol in combination with any drug, medication or sedative, or
  • Military or combat activities while serving in the armed forces, National Guard or organized reserve corps in any state, country or international authority.

For Individual coverage in the states of IA, ID, KY, LA, MD, ME, MI, MS, MT, NC, NM, NV, OK, RI, SC, TX, UT, VT, WA and WI, the exclusions are as follows. Some of these states have exceptions to these exclusions. See below for the exceptions. (Form Number: D497/17-6456 Series) No benefit shall be paid for Loss or Injury that is caused by, results from or contributed to by:

  • An intentionally self-inflicted Injury, suicide, or any attempt at suicide, while sane or insane;
  • Any active participation in a riot, insurrection or war, either declared or undeclared (in UT, participation must be voluntary; in OK, any active participation in a riot, insurrection or war, either declared or undeclared, when serving in the military or an auxiliary unit thereto; in MD, any active participation in war, either declared or undeclared; does not apply to NC);
  • The Covered Person’s taking or using (in MT, must be voluntarily) any narcotic, barbiturate or any other drug or medication unless taken or used as prescribed by a Physician (in LA, The Covered Person’s taking or using any narcotic or barbiturate, unless taken or used as prescribed by a Physician; in MD, the Covered Person’s being intoxicated or under the influence of any narcotic unless administered on the advice of a physician; in SC, the Covered Person being drunk or under the influence of any narcotic unless taken on the advice of a Physician; does not apply to NV and WA);
  • The Covered Person’s blood alcohol level being .08 percent weight by volume or higher (in ID, the Covered Person’s alcohol intoxication, as defined in the state where the accident occurred; in MT, consumption of alcohol must be voluntarily; in OK, The Covered Person’s alcoholism; does not apply in MD, NV, SC and WA);
  • The Covered Person operating or riding in any kind of aircraft, except as a fare-paying passenger on a regularly scheduled commercial flight;
  • The Covered Person committing or attempting to commit a felony or an assault or being engaged in an illegal activity (in UT, participation must be voluntary; in ID, the Covered Person’s participation in a felony; in MD, illegal occupation);
  • Sickness, disease, bodily or mental infirmity or their medical or surgical treatment including diagnosis (except bacterial infections which result from an Injury) or mental disease or disorder;
  • Voluntary gas inhalation or poison voluntarily taken (except accidental ptomaine poisoning in NC), administered or inhaled (does not apply to ID);
  • Taking alcohol in combination with any drug, medication or sedative (does not apply to ID, MD, NV, OK and WA), or
  • Military or combat activities while serving in the armed forces, National Guard or organized reserve corps in any state, country or international authority.
